Description (Service) Supports for older adults, and adults with disabilities to help them remain independent in their own homes for as long as possible * comprehensive community, health and in-home services are provided by nurses, personal support workers and volunteers, programs include: 
Caregiver support circle - a free program for adult caregivers at home caring for another adult (family member or friend) in the Grey-Bruce area, see separate listing for details 
Community Nursing - General nursing care including visiting and shift nursing * includes assessment, treatment and education of wound and ostomy care, education for people with diabetes, pain and symptom management, palliative care, TB skin testing clinics * various locations throughout Grey and Bruce, contact for details 
Falls Prevention and Group Exercise Classes for Seniors - Group exercise and falls prevention classes for older adults and people with mobility issues and chronic conditions * aims to improve strength and balance to prevent injury and falls * see separate listing for details 
Home At Last Program - Assists older adults returning home from a hospital stay  
Supportive Care Program - Hospice Visiting Service - Companionship, respite care, and emotional support to people and their family/caregivers dealing with a life-limiting or life-threatening illness * bereavement support for anyone who has experienced the death of someone close * services can be provided at home, or in hospitals and long-term care homes * palliative care education  
Supports For Daily Living - Care for older adults with chronic illness or cognitive impairment who require assistance with independent living
