Description (Service)

Compassionate care for individuals with life-limiting illness and their families-at Campbell House and in the communities of South Georgian Bay.

Services:

Campbell House - 10 bed palliative care residence providing:

  • End-of-life care
  • Short stay palliative respite
  • Pain and symptom management

In-Home Visiting Volunteer Support

  • Emotional support for individuals living with life-threatening illness
  • Support for caregivers and bereaved individuals (in person or by phone)

Professional Grief and Bereavement Support

  • Individual and group counselling available via phone, Zoom, or in person
  • Delivered by a team of social workers and psychotherapists

Drop-In Grief Support Groups

  • Peer-based support for anyone grieving a loss
  • Helps foster connection and build a personal support network

Community Programming

  • Grief and Bereavement Support
  • Caregiver Support Groups
  • Advance Care Planning
  • Education and Resources
  • Living Wish Program
    • Helps terminally ill patients realize one final dream or wish
    • Brings meaning and joy to the end-of-life journey

Shared Care program

  • Offered to support patients and caregivers as they navigate the healthcare system.

Hours Campbell House (for patients and families): Mon-Sun 24 hours
Hospice Georgian Triangle Programs and Administration Office: Mon-Fri 9 am-5 pm
The Hospice Georgian Triangle Foundation Office: Mon-Fri 9 am-5 pm, summer hours (effective until Labour Day): Mon-Thu 8:30 am-5:30 am

Eligibility Ages: 18 year(s) and up Campbell House Admission
  • Life-limiting illness with a prognosis of three months or less
  • Must not be seeking curative treatment
  • Referral required through Ontario Health atHome

  • Available to anyone in South Georgian Bay living with a life-limiting illness, their caregiver(s), or anyone grieving a loss
Open to individuals of all ages

Referrals for caregivers, bereaved individuals seeking grief support/counselling or community members living with a life-limiting illness and grieving children for support and programming can be made via self referrals or by other health care and support organizations.
