Programs Available:

• Lung Health Program: focus on educating asthma and/or Chronic Obstructive Pulmonary Disease (COPD) patients and their caregivers.
• Smoking Cessation Program: support for those ready to quit and those thinking about making a small change.
• Walking Program: led by a Registered Nurse & a Respiratory Therapist; program runs twice weekly for 6 weeks.
• Aging Well Program (RN led program): provide support to at risk seniors through system navigation; early identification, treatment and care coordination of patients with cognitive impairment.
• Diabetes Program (Interprofessional team program): improve health and well being of diabetic patients through regular monitoring; provide DM education.
• Women's Health and Wellness Program: focus on health promotion and disease prevention.
• Dietician: helping patients achieve their health goals through diet and lifestyle interventions.
• Mental Health Services (SW led program): learn coping skills; reduce symptoms of distress; improve quality of life.
• Preventative Care (admin led program): patients are contacted for prev care (Paps, Mammograms, FIT testing)
• Hypertension and Blood Pressure Monitoring Program: provides accurate, non-invasive readings, 24 hour BP Monitoring and personalized support to help with understanding results and maintain healthy levels.
• Foot Care Program (Nurse led program): comprehensive foot care services address a wide range of concerns-from routine nail care and callus management to diabetic foot assessments.
